Once your completed application has been received, you will be notified in writing of your eligibility status within 21 days.  Please note that the eligibility determination process may include an in-person interview and/or functional assessment.  If this is the case, you will be contacted to schedule an appointment. Persons found eligible will receive a Lift Line ID card.
